NorthWest CDC: “Cohesion – Tri-Generational HomeCare @ North West”

Since 2014, student volunteers from Tri-Generational HomeCare @ North West have been visiting seniors at home – to befriend them, help monitor vital signs, manage medications…

…but how do you visit seniors in a pandemic? Project Directors Lim Jia Yin and Ong Yuki share how they and their fellow volunteers have continued to make sure our vulnerable seniors in the North West District continue to be cared for in this time of COVID-19.

❤️ What does Tri-Generational HomeCare @ North West do?
Tri-Generational HomeCare @ North West volunteers conduct home visits with seniors to help with their medical and psychosocial needs. These seniors have been identified to be at risk of hospital readmission. Each team usually consists of a Team Leader (a university student) and 4–5 secondary school students.

❤️ How did Tri-Generational HomeCare @ North West start?
This is a ground-up initiative led by NUS students from Medicine, Nursing, Pharmacy, Social Work, Physiotherapy, and Occupational Therapy faculties, who are passionate about caring for the community and keen to put their academic knowledge to practical use. Tapping on local networks, the CDC connects the tertiary students, secondary school students and nurses at Khoo Teck Puat Hospital in efforts to reduce the number of elderly patients, who are identified as “frequent flyers” from being re-admitted to hospitals.
